Exosomes (or extracellular vesicles – EV’s) are tiny, microscopic bundles of signalling molecules that allow communication between cells such as fibroblasts, keratinocytes and immune cells. Topical exosome treatments are a major advancement in the field of regenerative medicine. With billions of exosomes in each vial, exosomes enhance the growth of hair follicles when applied to the scalp and work to decrease inflammation after needling or skin resurfacing treatments over the face and neck.
What are Exosomes?
Exosomes are bioactive nanoparticles derived from stem cells that carry messages between our cells. Exosomes facilitate the exchange of proteins, growth factors, and genetic material, and allow cell-to-cell communication, which is critical for our skin tissues to function properly. When applied topically to our skin, exosomes have anti-inflammatory properties and can tell our cells to heal, regenerate, and can stimulate our hair follicles.
At RiverEnd Aesthetics we use ExoCoBio ASCE+ exosomes. These bioengineered exosomes are extracted from healthy, human mesenchymal stem cells and rigorously tested and purified. ExoCoBio exosomes contain stem cell derived lypholised exosomes, growth factors, peptides, coenzymes, amino acids, minerals and vitamins.
ExoCoBio exosomes have the strongest clinical data and highest safety profile of all the EV’s available worldwide. There are many different types of extracellular vesicles being manufactured these days and clinical trials emerging in many fields of medicine. ExoCoBio also develop exosomes to treat dermatitis, kidney disease, immunological conditions and inflammatory bowel disease. We are lucky in the aesthetic industry to be allowed to harness the anti-inflammatory and regenerative powers of exosomes applied to the skin.
What areas can be treated with Exosomes?
Hair Loss
Exosomes can be applied to the scalp or beard to areas where hair follicles are thinning or perhaps lying dormant. At RiverEnd Aesthetics we couple exosome applications with Alma Impact TED treatments to enhance the permeability of the skin and increase penetration of the exosomes, for pain-free, needle-free hair restoration. When applied to the scalp or beard, exosomes can reactivate hair follicles, hydrate and nourish the skin, decrease hair shedding, and promote a normal, healthy, hair cycle.
Face & Neck
Exosomes can minimise post-treatment inflammation and encourage cellular repair processes. We will often apply a layer of exosomes to your skin immediately after skin resurfacing treatments with the Alma Opus Plasma device, and provide you with the rest of the vial of exosomes to take home. When applied to the face and neck, exosomes can minimise the appearance of fine lines and wrinkles, hydrate and nourish the skin, and promote a more even skin tone.

How do Exosomes work?
Exosomes carry messages between cells, encouraging repair and regeneration processes. There are multiple ways exosomes work:
– Anti-inflammatory effects
Exosomes contain tiny bioactive signalling molecules such as proteins and microRNA, which can modulate the body’s immune and inflammatory responses. By reducing inflammation, exosomes help to create the right environment for tissue repair and regeneration.
– Cellular signalling
Exosomes carry signalling molecules, such as growth factors, that can influence neighbouring cells (“cell-to-cell communication”). Exosomes can promote the proliferation and migration of fibroblasts and keratinocytes, which are essential for the repair of skin cells.
– Stimulating collagen
Exosomes can stimulate fibroblast cells to produce collagen, which is a protein that gives the skin elasticity and firmness. Increased collagen production can help to improve the skin’s overall tone and texture.
– Promoting angiogenesis
Exosomes can encourage the formation of new blood vessels (angiogenesis). With increased blood flow to the treated area, the tissues receive more oxygen and nutrients, which helps to facilitate healing of the skin or hair follicles.
– Enhanced cell turnover
Exosomes can promote shedding of damaged skin cells, and increase the production of healthy new cells. This process is vital in tissue repair and regeneration.
Results
When using exosomes for hair restoration, results vary between patients and depend on the underlying cause and severity of hair loss. Usually, results can start to be seen approximately 1 month after commencing treatment sessions.
When applying exosomes after skin resurfacing treatments or injectable skin boosters, results appear within minutes to hours, as the exosomes work to immediately minimise inflammation and redness.
How many treatments will I need?
For hair restoration, depending on the severity and underlying causes of your hair loss, an initial treatment course of 6-12 exosome applications is generally recommended, around 2-4 weeks apart. Maintenance treatments are strongly recommended and the frequency of these sessions will be determined by your clinical response.
What are the benefits of Exosomes?
There are many benefits of exosomes when applied to the face, neck and scalp, these include:

Do Exosomes hurt and is there any downtime?
The application of exosomes is entirely painless. Exosomes are packaged as a dry powder and reconstituted with ASCE+ dilutent, or saline, into a clear, watery serum.
Exosomes work to minimise inflammation, so they actually help to reduce the total downtime after other skin treatments.
Are there side effects? What can I expect after a treatment?
There are generally no unwanted side effects after the application of exosomes. There is a remote possibility of unknown risks and complications of exosome treatment that have not been discovered yet, although this is extremely unlikely.
After exosome treatment for the face and neck, we will usually provide you with the rest of the vial to apply at home. The exosome liquid should be repeatedly applied over 3 hours; exosomes are thought to become inactive 3 hours after the vial is mixed.
Exosome application to the scalp for hair restoration can leave your hair a bit damp for an hour or so. We recommend avoiding washing/showering the scalp until the day after treatment, or at least 6 hours after exosome application.
What precautions should I take pre- and post-treatment?
There are no specific pre- or post-treatment precautions required.
